Clinic/Services Request Form
Our waitlists for spay/neuter and wellness are currently closed. Due to the high demand and long wait, we have temporarily paused adding new animals to the spay/neuter waitlist. We encourage anyone looking for spay/neuter services to check out these other options. We will reopen the spay/neuter waitlist once we are caught up with the current backlog of requests. We will reopen the wellness waitlist in the spring when we resume clinics. Please check out these other options for wellness in the meantime. If you are already on the spay/neuter or wellness waitlists, someone from Leech Lake Legacy will reach out to you by phone, text or email in 2026 when we reach your spot on the list. Please continue to use this form to submit requests for pet surrender.
